Home Health Physical Therapist

Responsibilities

• Perform evaluations and develop personalized care plans for patients with chronic conditions
• Provide evidence-based treatments to enhance mobility, safety, and independence at home
• Educate patients and caregivers on treatment plans and safety measures
• Document all care activities in Electronic Medical Records (EMRs)
• Collaborate with an interdisciplinary team to ensure comprehensive patient care

Qualifications & Skills

• Licensed Physical Therapist with relevant credentials
• Strong clinical assessment and treatment skills
• Excellent communication and patient education abilities
• Ability to work independently and as part of a team
