Home Health Physical Therapist

About the role
As a Home Health Physical Therapist, you will plan and administer prescribed physical therapy treatment for patients in their homes to help them attain the highest level of physical function.
Responsibilities
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement a rehabilitation program consistent with the physician's Plan of Treatment and the goals of the patient team.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Document patient observations, interventions, and evaluations promptly and thoroughly.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
Requirements
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
Qualifications
Preferred: One year of experience as a Physical Therapist.

Pay
- Per Visit/Unit Rate: $76.00 - $106.00.
- Full-time (40 hours per week) pay range: $111,600 - $153,600 per year (varies based on geographic location, skills, knowledge, experience, education, and certifications).
Schedule
Scheduled weekly hours: 40.
Additional Information
- This role is considered patient-facing and requires tuberculosis (TB) screening.
- This role requires a valid state driver's license and maintenance of personal vehicle liability insurance (minimum required limits or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher).
