Home Health Physical Therapist Assistant

About the role

As a Home Health Physical Therapist Assistant at CenterWell Home Health, you will help patients regain strength, mobility and independence from the comfort of their homes. You will administer the physical therapy treatment plan to patients in their homes as developed by the Physical Therapist, focusing on functional improvement to empower individuals to overcome limitations and enjoy a better quality of life.

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, heat, light, cold, water, and electrical modalities to patients.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of any equipment failures, lack of supplies, or staff or client changes.
• Apply concepts of infection control and universal precautions in performing patient care activities.

Required Qualifications

• Current state license to practice Physical Therapy.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population.
• Read, write, and speak fluent English.

Preferred Qualifications

• One year of experience as a Physical Therapy Assistant.

Pay

Pay Per Visit/Unit Rate: $41.00 – $57.00

Estimated annual full-time pay range: $64,000 – $87,500

Schedule

Scheduled Weekly Hours: 40
