Schedule
Full‑time position, Monday – Friday. As an OT Case Manager you will manage your schedule during office hours and travel to clients’ homes for 1–3 hour visits.
Coverage Area
Reports to the Rochester, MN office and provides services in Rochester and surrounding cities.

Pay
Base salary range $70,000 – $100,000 per year plus productivity, compensation and benefits.
Responsibilities
- Perform initial and periodic assessments of clients, implement interventions, and evaluate effectiveness.
- Establish reasonable treatment goals, keep care plans current, reassess clients, and make recommendations.
- Provide occupational therapy treatments, procedures, evaluations, and diagnostic tests under physician direction.
- Translate exercises into functional activities of daily living.
- Instruct clients in the use and care of special equipment when necessary.
- Develop plans for modifying equipment, appliances, and home environments.
- Prepare clinical and progress notes; report client response or condition changes to physicians and the Area Clinical Manager.
- Maintain appropriate records, including visit frequency and client response.
- Instruct and supervise COTAs, HHAs, LPNs, and RNs in the therapy program when appropriate.
- Provide in‑service education programs for company employees as requested.
- Participate in case conferences as indicated.
- Provide families with information, support, and encouragement; instruct families on home therapy programs.
- Assist individuals and families in positively adjusting to physical, mental, and social limitations.
- Participate in developing community resources to meet client needs.
- Maintain positive and effective communication with all employees and others; embrace a team concept.
- Maintain absolute confidentiality of all client, family, and employee information.
- Maintain a safe client environment and report any suspected abuse, neglect, or financial exploitation.
- Maintain productivity according to guidelines.
- Perform other related duties and responsibilities as assigned.
Physical Demands
- Heavy to very heavy work, including lifting clients, bending, stooping, stretching, and setting up equipment.
- Adaptability to a variety of environments and community settings.
- Ability to drive or use public transportation in all weather conditions.
- Exert force >100 lb occasionally, >50 lb frequently, and >20 lb constantly to move objects.
- Physical demand requirements exceed those for standard heavy work.
Qualifications
- Current Minnesota occupational therapist license without revocation, suspension, or restrictions.
- Graduate of an AOTA‑approved school of occupational therapy and AOTA‑certified.
- At least two (2) years of experience in a health‑care setting or equivalent; home‑care experience preferred.
- National Board of Occupational Therapy certification strongly recommended.
- Strong interpersonal communication and teaching skills.
- Strong organizational skills and ability to work independently.
- Excellent written and oral communication skills.
- Access to a dependable vehicle or public transportation for multiple daily business stops.
- Valid driver’s license and proof of car insurance when using a personal vehicle for company business.
- Proof of a negative Mantoux test or documentation of a negative chest X‑ray.
- Current CPR certification recommended.
- U.S. citizenship or valid Alien Work Permit.
- Disclosure of any conviction related to health‑service provision; such candidates will not be hired.
- Pass initial and ongoing background screenings (Minnesota Department of Health, Federal OIG List of Excluded Individuals and Entities, etc.).
