Home Health Occupational Therapist (OT)
About the role
Provide skilled, one-on-one therapy services in the comfort of patients’ homes, helping them regain independence with daily activities and safely navigate their environments.
Responsibilities
- Assess patient needs
- Develop individualized treatment plans
- Deliver evidence-based interventions
- Educate patients and caregivers to support functional independence and quality of life at home
Requirements
- Experience in home health, or transition from hospital, outpatient, acute care or skilled nursing settings
- Practicing at the top of your license with greater flexibility, autonomy and meaningful 1:1 patient relationships
Qualifications
- Current licensure as an Occupational Therapist in Michigan
- Passing scores on the National Board for Certification in Occupational Therapy (NBCOT) exam
Skills
- Strong clinical skills
- Effective communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of home health care practices and regulations
