About the role

Provide skilled, one-on-one therapy services in the comfort of patients' homes, helping them regain independence with daily activities and safely navigate their environments.

Responsibilities

• Assess patient needs
• Develop individualized treatment plans
• Deliver evidence-based interventions
• Educate patients and caregivers to support functional independence and quality of life at home

Requirements

• Experience in home health, or transition from hospital, outpatient, acute care or skilled nursing settings
• Practicing at the top of your license with greater flexibility, autonomy and meaningful 1:1 patient relationships

Qualifications

• Current licensure as an Occupational Therapist in Michigan
• Passing score on the NCLEX-RN
