Responsibilities
- Provide skilled physical therapy care, conducting comprehensive in-home evaluations and developing individualized plans of care for geriatric patients.
- Implement therapeutic interventions focusing on functional strengthening, gait training, balance, and fall risk reduction to improve patient mobility and independence.
- Manage your caseload by scheduling and conducting home visits based on your availability and patient needs.
- Collaborate with a supportive care team, including other disciplines and supervisors, to ensure coordinated patient care.
Requirements
- Valid California Physical Therapist license.
- CPR or BLS certification.
- Driver’s license and auto insurance.
- Physical and TB test (within the last year).
- Professional liability and business license (if applicable).
