About the role
Principle Choice Home Health is seeking a self-motivated Occupational Therapist to join our team. This position reports to the Director of Therapy and involves evaluating and treating home health patients with various impairments, establishing treatment plans, and collaborating with healthcare teams.
Responsibilities
- Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
- Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
- Provide a written evaluation and plan of care defining occupational therapy needs, goals, progress, and expected outcomes.
- Deliver occupational therapy services to comply with prescribed physician orders.
- Maintain plan of care by documenting client performance, modifying goals and treatment as needed, and determining discharge care plans as appropriate.
- Collaborate and educate patients and caregivers on home exercise plans following discharge.
- Coordinate services with healthcare team, physicians, parents/guardians, nurses, etc.
- Document according to policy and procedure, requiring minimal supervision or instruction.
- Follow policies and procedures, conforming to patient care standards, licensure regulations, and state laws.
- Maintain effective communication with staff, healthcare team members, and family.
- Supervise other personnel (home health aides, occupational therapy assistants) as applicable.
- Complete OASIS assessments where allowed by state professional practice and regulation.
- Other duties as assigned.
Requirements
- Degree in Occupational Therapy from an accredited university/program.
- Current state licensure in Oklahoma and in good standing per state requirements.
- Current Basic Cardiac Life Support (BCLS) required.
- 1-2 years recent Home Health experience preferred.
- Active Oklahoma driver's license.
- Current vehicle insurance and registration.
- Required vehicle.

Physical Requirements
- Moderately heavy physical exertion on a regular and recurring basis such as: extensive driving, assisting patient in transfer activities (wheelchair, to bed, to tub, to commode), and providing substantial support to individuals in ambulation.
- Use hands to finger, to handle or feel, and talk or hear.
- Stand, walk, reach with hands and arms, stoop, kneel, crouch, or crawl.
- Sit occasionally.
- Lift and/or move over 100 pounds occasionally.
