Home Health Nurse Clinical Manager (Broward County FL,)

Responsibilities

• Perform case management oversight, and coordination of care for all skilled disciplines.
• Closed communication with social worker and veterans division.
• Cook up patient admission with other members of the healthcare team.
• Provide nursing care and treatment of home care patients in accordance with the plan of treatments and approved authorizations.
• RN will supervise all non-skilled caregivers.

Qualifications

• Current RN Florida professional license.
• Minimum of one year experience with Medicare home health case management.
• Must have excellent time management skills to prioritize and organize workload.
• Positive attitude and strong work ethic.
• Excellent communication and customer service skills.
• Experience with Kinser Wellsky a plus.
