Home Health Nurse

Job Description

Captures the total Plan of Care for the patient in the Home Care setting by assessing, planning, implementing, and evaluating. Leads the Health Care team by collaborating with other team members to maintain standards of professional nursing practice in the Home Care setting.

Education And Credentials

Requires an Associate's Degree. Current license required. 1-5 years of relevant experience required.

Working Conditions

Full Time Regular, Weekend/Holiday Requirement: Yes, On Call Required: Yes, With Rotation: Scheduled Work Hours: 8a-4:30p, Work Arrangement: Onsite, Union Code: U20 - CWA 1122 VNA RN & Clerical.
