A Home Health Registered Nurse develops and oversees nursing care plans that individually address each patient's needs. Our RNs go beyond that and care for the patient's mind, body and spirit while supporting their families and caregivers as well. Home healthcare requires a special balance – you are the bridge between the physician and the home.

What You Can Expect

• Sign-on bonus available (ask your Talent Acquisition Advisor if you are eligible)
• Minimum start pay $34.00/hour
• Full Time, Part Time & PRN positions available
• Makes initial nursing evaluation visit to communicate with physician and determine a plan of care
• Practices nursing according to Franciscan Alliance and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations
• Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families
• Collaborates with patients and their families, as well as members of a multidisciplinary team, to ensure optimal patient outcomes and enhancement of the patient experience
• Assesses, collects, and analyzes comprehensive objective, subjective, psychosocial, and cultural data pertinent to patient condition to determine the nursing diagnosis, patient needs and plan of care
• Develops an appropriate plan of care based on objective and subjective data, observations, and communications from patient and family and continuously reviews and revises the plan of care as patient condition warrants
• Prioritizes and implements the identified plan, coordinates care delivery, employs interventions to improve patient outcomes while providing a safe, respectful, and ethical practice environment
• Evaluates the individualized plan of care and progress towards attainment of outcomes in collaboration with patient and family utilizing available data and resources
• Communicates timely changes in patient's assessment/ongoing reassessment to providers that warrant consideration and/or a change in the plan of care and maintains and coordinates communication with all members of the multidisciplinary team to facilitate safe transitions and continuity in care delivery
• Maintains continuity among nursing teams by documenting the assessment, plan of care, interventions, and responses to care in the electronic medical record in a timely manner
• Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand the condition, medications, and self-care skills; answering questions
• Maintains a clean and safe work environment by handling and clearing instruments, equipment, and supplies in accordance with infection-control policies and protocols
• Administers and stores medication according to controlled substance regulations
• Ensures operation of equipment by completing preventive maintenance requirements as per manufacturer's instruction or calling for repairs when required
• Maintains nursing supplies inventory by checking stock to determine inventory level, anticipating needed supplies and placing orders for required inventory
• Supervises clinical care provided by LPNs and Home Health Aides
• Serves in an on-call rotation and provides patient care outside of regular business hours

Qualifications

• Associate's Degree in Nursing - Required
• Bachelor's Degree in Nursing - Preferred
• 1 year Acute Care Experience - Required
• 2 years Home Health Experience - Preferred
• 2 years Long Term Care Experience - Preferred
• Registered Nurse (RN) - State Licensing Board - Required
• Basic Life Support Program (BLS) - American Heart Association - Required
• Driver's License - Required
• Auto Insurance - Required
