Role Overview
The Medical Social Worker provides personalized emotional and social care to home health patients, working within interdisciplinary teams to assess needs, develop care plans, and connect patients with community resources. They supervise the Social Worker Assistant and ensure compliance with policies, standards, and regulations.
Key Responsibilities
- Evaluate patients' social and emotional well-being related to their health conditions.
- Assess relationships, financial resources, and community support systems.
- Facilitate access to community resources to support recovery.
- Communicate effectively with patients, families, and healthcare team members.
- Monitor ongoing patient and family needs, providing guidance on treatment and coping strategies.
- Participate in care coordination and serve as a resource for team members.
- Evaluate and supervise the Social Work Assistant to ensure quality services.
- Maintain documentation including visit reports, progress notes, and case conferences.
Qualifications & Skills
- Minimum one year of healthcare social work experience, home health or hospice preferred.
- Valid licensure as required by state law.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Knowledge of discharge planning, community resources, and medication administration.
- Strong organizational, prioritization, and stress management skills.
- Fluent in English, with flexibility for local travel.

- Participation in TB screening and driver safety programs.
- Compliance with licensure, continuing education, and safety policies.
