Position Overview

Aveanna Healthcare is expanding through acquisitions and seeks a Social Worker to join its dynamic home health care team. Key responsibilities include evaluating and coordinating social work services, developing care plans, arranging community resources, and ensuring client progress.

Essential Job Functions

• Assist clients in understanding social and emotional factors affecting health.
• Develop and implement care plans based on clinical diagnoses and client input.
• Arrange implementation of care plans and link clients to community resources.
• Review client progress to assess service effectiveness and set measurable goals.
• Use appropriate medical, social, diagnostic, and psychological terminology.
• Investigate and address client and employee concerns.
• Instruct clients, family members, and caregivers to optimize functioning.
• Coordinate agency services and ensure caregiver training.
• Complete required documentation and transfer medical data to physicians.
• Provide input to the Medicare Director on policy changes.
• Maintain accurate client data and participate in service planning meetings.
• Comply with policies, procedures, and regulatory requirements.

Requirements

• Current state license as required by state law.
• At least one year of medical social work experience.
• Master’s degree in Social Work.
• Must comply with health-related requirements, including vaccinations, as applicable.
