Home Health LPN / Licensed Practical Nurse Healthcare Support is actively seeking a Home Health Licensed Practical Nurse to fill an opening for a top healthcare provider located in Scranton, PA. Daily Responsibilities for the Home Health LPN / Licensed Practical Nurse: Home healthcare nurses provide follow-up care to patients after they have been discharged from a hospital or rehabilitation, skilled nursing, or long-term care facility Provide nursing visits in accordance with the written plan of treatment signed by the physician and all subsequent orders that authorize and direct changesProvide (on a daily basis) paperwork as required by agency to maintain accurate and complete medical records Pay Details: Position is Pay Per Visit however annual salary ranges between $55-62K for 32 hours per weekShift: 32 hours a week – Monday – Friday | On call rotation is 1 week on every month Sun-Sat. Required Qualifications for the Home Health LPN / Licensed Practical Nurse: Currently Licensed Practical/Vocational Nurse in PennsylvaniaMust possess and maintain valid CPR certification while employed in a clinical roleAt least 1-year acute care experience as an LVN (Home Health, Med-Surg, ED, SNF etc.)Experience with IVs, Wound Care, and Disease Management (diabetes, CHF, liver disease, COPD)Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and in good working order Benefits for the Home Health LPN / Licensed Practical Nurse: Medical, Dental, VisionFSA & HSA Options401(k) with match6 Paid Holidays + Eligible for 2 floating holidays based on date of hire 15 Days PTO (1-5 years) | 20 Days (5-10 years) | 26 Days (10+ years) Interested in being considered?
