Home Health Licensed Practical Nurse - Per Diem

Role Overview: A Home Health LPN provides personalized nursing care in patients' homes, working under the supervision of an RN. They collaborate with healthcare teams to implement care plans, educate patients and families on disease management and self-care, and maintain accurate documentation of care, interventions, and outcomes. Key Responsibilities Deliver skilled nursing care onsite, ensuring patient comfort and safety.Coordinate with physicians, clinical managers, and families regarding patient needs and status changes.Participate in care coordination, discharge planning, and patient education. Qualifications & Skills Valid nursing license in the practicing state.Current CPR certification.Reliable transportation, valid driver’s license, and auto insurance.At least two years of clinical experience as an LPN/LVN. Preferred Experience: Background in Home Health or Hospice settings is advantageous. Additional Details: Compensation ranges from $58,300 to $80,300 annually, with pay per visit/unit rate between $33.00 and $47.00. The role involves TB screening and driver safety requirements.
