Home Health Licensed Practical Nurse-Per Diem
About the Role
As a CenterWell Home Health nurse, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love.
Territory: Goldsboro, Rocky Mount, Raleigh.
Responsibilities
- Provide preventative nursing care to patients, working onsite in their homes, under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and their families on disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient care, including documentation of patient visits, interventions, and outcomes.
- Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes.
- Participate in care coordination activities and discharge planning.
Requirements
- Current nursing license in the practicing state.
- Valid driver’s license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.
This role is considered patient-facing and requires Tuberculosis (TB) screening. You must maintain personal vehicle liability insurance in accordance with your residing state’s minimum required limits or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher.
Pay
- Per Visit/Unit Rate: $33.00 - $47.00
- Full-time (40 hours per week) starting base pay range: $53,000 - $73,000 per year
