Home Health Licensed Practical Nurse

Responsibilities

• Assist with assessment, planning, implementation, and evaluation of patient care according to age and diagnosis.
• Assist with patient care management in coordination with a multidisciplinary team.

Requirements

• Education: Graduate of an accredited or approved school of practical nursing.
• Licensure / Certifications: Current license to practice practical nursing in the State of North Carolina. CPR certification required.
• Experience: Minimum one year practical nursing experience required. Home health experience preferred.
• Essential Technical / Motor Skills: Hand/eye coordination for injections, computer use, and patient care equipment. Ability to communicate and be literate in English. Ability to manipulate patient care equipment and properly transfer and guard patients.
• Interpersonal Skills: Ability to develop positive interactions with patients, families, physicians, and staff.
• Essential Physical Requirements: Ability to transfer and maneuver objects weighing at least 50 pounds. Frequent pushing, moving, lifting of patients. Positioning, bathing, and ambulating patients. Occasionally requires reaching overhead, stair climbing, and fine motor manipulation.
• Essential Mental Abilities: Ability to assess patient conditions, formulate care plans, select interventions, evaluate responses, and explain/teach about conditions and recovery. Requires higher-level mental faculties, short- and long-term memory, prioritization, learning new skills, and adapting to supportive services.
• Essential Sensory Requirements: Visual assessment of patients and use of sight for implementing and evaluating care (e.g., changing dressings, phlebotomy, regulating IVs). Use of hearing to auscultate lung and bowel sounds, hear alarms, and communicate effectively.
• Exposure to Hazards: Noise, bloodborne pathogens, body fluids, needle puncture wounds. Potential exposure to dangerous animals, traffic hazards, and safety threats during home visits. May encounter temperature changes and weather extremes.
• Must have a valid North Carolina driver’s license and an operational vehicle.

Schedule

Variable Monday–Friday, weekends and holidays as needed.

Adolescents, adults, geriatrics, and pediatrics.
