Home Health Licensed Practical Nurse

About the role

Provide personalized nursing care in patients' homes, collaborating with healthcare teams to support recovery and independence. Implement individualized care plans, educate patients and families on disease management and self-care techniques, and maintain accurate documentation of patient visits, interventions, and outcomes. Coordinate care activities, participate in discharge planning, and communicate promptly with team members about patient needs and status changes.

Requirements

• Current state nursing license
• Valid driver’s license and reliable transportation
• CPR certification
• At least two years of clinical LPN/LVN experience
• Experience in home health or hospice (preferred)
• Adherence to TB screening and driver safety programs

• Skilled in patient care, documentation, and care coordination
• Proficient in patient education
• Ability to work independently and as part of a team
• Strong communication and organizational skills
