Responsibilities

• Provide direct in-home care to elderly, chronically ill, and disabled clients
• Perform assessments and monitor vital signs
• Administer medications and support wound care
• Implement care plans under RN and physician supervision
• Educate patients and families about health management
• Document care activities accurately in Electronic Health Records (EHR)
• Coordinate with a patient-focused, mission-driven team

Qualifications

• Valid LPN licensure
• Compassionate, patient-centered approach
• Strong communication and documentation skills
• Ability to work independently and as part of a team
• Willingness to learn and grow professionally
