About the role
Central Vermont Home Health & Hospice is a community-focused, not-for-profit organization dedicated to enhancing the well-being of central Vermonters through exceptional home health and hospice care.
Responsibilities
- Determine patient care needs and develop goal-oriented care plans in consultation management.
- Ensure accurate and timely documentation of patient information, adhering to HIPAA regulations.
- Work autonomously under RN direction and utilize supervisory support when necessary.
- Empower clients to explore care options and alternative plans to achieve desired outcomes.
- Participate in on-call rotation and respond to calls promptly.
Requirements
- Valid Vermont LPN License
- Minimum of one year progressive clinical experience.
- Home health and wound care experience preferred.
- Valid Vermont Driver’s License
