Home Health Licensed Practical Nurse

About the role

As a CenterWell Home Health nurse reporting to the Clinical Manager, you will provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love. The position serves the Raleigh, Rocky Mount, and Goldsboro coverage area.

Responsibilities

• Provide preventative nursing care to patients in their homes under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Educate patients and families on disease management, treatment options, and self‑care techniques.
• Maintain accurate records of patient care, including documentation of visits, interventions, and outcomes.
• Maintain timely contact with patients, physicians, the clinical manager, and other healthcare team members regarding patient or family needs and status changes.
• Participate in care coordination activities and discharge planning.
• Comply with Humana’s tuberculosis (TB) screening program as a patient‑facing role.
• Maintain a valid driver’s license, auto insurance, and reliable transportation in accordance with Humana’s driver safety program (state minimum liability limits or $25,000 bodily injury per person/$25,000 per event/$10,000 property damage, whichever is higher).

Requirements

• Current nursing license in the state of practice.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• Minimum two years of experience as an LPN/LVN in a clinical setting.

Qualifications

• Preferred: Nursing experience in a home health or hospice setting.
