Role: Licensed Practical Nurse providing compassionate home health care to pediatric and adult patients. Responsibilities Deliver skilled nursing services in patients’ homes and community settings.Perform medication administration, wound care, and monitor vital signs and clinical status.Follow individualized care plans and document all care in electronic records.Communicate patient updates and changes to RNs and physicians. Qualifications & Skills Valid LPN license.Strong clinical skills and ability to provide personalized patient care.Excellent communication and documentation abilities.Ability to work independently and as part of a team.
