Home Health Field Registered Nurse (RN)

About the role

Provide direct patient care in a home health setting. Perform assessments, develop care plans, administer medications, educate patients and families, and coordinate care with other healthcare providers.

Responsibilities

• Assess patient needs and develop individualized care plans
• Administer prescribed medications and treatments
• Educate patients and families on self-care and disease management
• Coordinate care with other healthcare professionals
• Document patient progress and communicate with supervising nurses

Requirements

• Bachelor's degree in Nursing (BSN)
• Current California RN license
• Passing score on the NCLEX-RN exam

Qualifications

• At least 1 year of acute care nursing experience preferred
• Experience in home health or similar field
• Strong communication and interpersonal skills

Skills

• Effective problem-solving and critical thinking skills
• Ability to work independently and manage time effectively
• Proficiency in electronic medical records (EMR) systems
