Home Health Field Physical Therapist (PT)

About the role

The primary focus is on providing physical therapy services to patients in their homes. This includes assessment, treatment planning, and patient education.

Responsibilities

• Assessing patient needs and developing individualized treatment plans
• Administering physical therapy treatments including exercises, modalities, and therapeutic activities
• Documenting progress and communicating with referring physicians and other healthcare providers
• Maintaining compliance with regulatory standards and insurance requirements

Requirements

• Bachelor's degree in Physical Therapy from an accredited program
• Current California Physical Therapy License
• At least 1 year of relevant clinical experience
• Excellent communication and interpersonal skills

Qualifications

• Strong manual therapy skills
• Experience working with diverse populations
• Ability to work independently and as part of a team

Skills

• Proficient in computer software used for documentation and scheduling
• Knowledge of home health care protocols and regulations
