Home Health CNA - Certified Nursing Assistant

Responsibilities

• Provide compassionate in-home support to seniors and individuals with chronic or disabling conditions
• Assist with personal care, mobility, medication reminders, meal preparation, and light housekeeping
• Monitor and report any changes in clients' conditions to ensure safety and well-being
• Collaborate with nurses and care coordinators to follow individualized care plans
• Help clients maintain independence and dignity in their home environment
