Responsibilities

  • Ensure patient care is coordinated and managed appropriately by a team of RN Case Managers and LVNs.
  • Serve as the first point of escalation for support, ensuring timely access to resources.
  • Identify, develop, design, and implement nursing educational programs to enhance clinical excellence and staff development.
  • Assess staff learning needs and formulate specific plans and goals for ongoing evaluations.
  • Coordinate at least bi-weekly Case Conferences with the clinical team to discuss patient progress and revise care plans as needed.
  • Review and evaluate each case, assisting clinicians in establishing therapeutic goals, setting priorities, and developing a plan of care.
  • Provide direct support to RN Case Managers in the field, handling patient activities and escalations.

Requirements

  • Two years' management or supervisory experience within the past five (5) years in a health care setting, preferably in Home Health.
  • Graduate of an accredited school of nursing, with a Bachelor’s Degree preferred.
  • Current CA Registered Nurse License (RN).
  • Current Basic Life Support (BLS) Card.
  • Valid CA Drivers License with an acceptable driving record.

  • Experience with patient care coordination and supervision.
  • Knowledge of home health care practices and regulations.

$155,000.00 - $170,000.00 annually, plus a generous annual bonus of $40,000.00, paid quarterly.

Schedule

This role is hybrid, combining field work and on-site duties. The Sacramento Office requires 2 days per week, while fieldwork involves 3 days per week (averaging 12+ per week).
