What You'll Do
- Delivers skilled nursing care to patients in their home in accordance with individualized care plans
- Initiates appropriate preventative and rehabilitative nursing procedures, making necessary referrals to community services
- Captures ongoing medical documentation, writing OASIS assessments as appropriate
- Prepares clinical and progress notes, submitting all required paperwork daily
Key Responsibilities

What We Are Looking For
- Current RN license in the state of Massachusetts
- Strong technology skills
- Reliable transportation, valid driver's license, auto insurance
- Strong clinical assessment and documentation skills
