Key Responsibilities
- Adhere to care plan to ensure safe and competent care for the patient.
- Absorb and implement the care plan to ensure safe and competent care for the patient.
- Assist patients with personal hygiene and support the maintenance of a clean, safe, and healthy environment.
- Help patients with ambulation as prescribed by the physician and approved or supervised by a clinician or therapist.
- Support therapy personnel as needed during rehabilitative processes.
- Encourage patients to achieve maximum independence according to the nursing care plan.
- Promote mental alertness by engaging patients in activities of interest.
- Provide emotional and psychological support to patients and household members, fostering trust and confidentiality.
- Report any changes in the patient’s condition or home environment to the RN or clinical manager.
- Complete visit reports promptly and accurately within the electronic medical record system.
- Confirm daily visit schedules to ensure seamless coordination with other staff.
- Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
- Understand and adhere to the agency’s process for managing and addressing customer complaints.
- Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.
- Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge.
Requirements
- Must have a CNA or HHA license.
- Must have completed a minimum of 75 hours of training, which includes an introduction to personal care services.
- Must be free from health problems that may be injurious to patient, self, and coworkers and must present appropriate evidence to substantiate this.
- Ability to comprehend the basics of personal care, housekeeping, and meal preparation.
- Must understand and respect patient needs including ethics and confidentiality of care.
- Must have a valid driver’s license and access to a reliable and insured vehicle.
- Ability to visit homes that may not be wheelchair accessible.
- Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
- Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
