Responsibilities
- Provide personal care and supportive services to individuals in their homes.
- Deliver health-related services as outlined in the plan of care, authorized by a physician, and established by a registered nurse or licensed therapist, under the direction of the Aide Matrix established by the NYS Department of Health.
- Assist with personal hygiene, housekeeping, and other supportive tasks for patients with healthcare needs.
- Immediately report changes in a client’s condition to the supervising professional.
- Ensure the safety of the client, client’s family, and/or the employee.
- Maintain HIPAA compliance.
- Complete and submit necessary patient documentation, including charting tasks and observations, on an Android device.
- Attend ongoing in-service programs in a timely manner to maintain certification.
- Perform all other duties as assigned.
Requirements
- High School diploma or GED preferred.
- Completion of an approved PCA or HHA certification program as designated by the NYS Department of Health.
- Valid Driver’s License and reliable transportation.
- Proof of completion (original HHA or PCA Certificate) of an NYS Dept of Health accredited training program.
- Must have worked in a certified HHA or PCA capacity within the last 24 months (verification via the NYS DOH’s Home Health Aide Registry).
Work Environment
The HHA/PCA works primarily in a non-office setting and may be exposed to outdoor conditions. The role is classified as medium work, involving exertion of up to 50 pounds of force occasionally and/or negligible force frequently or constantly to lift, carry, push, pull, or move objects, including the human body. The position involves sitting and standing.
