About the role
We're seeking a compassionate and skilled Home Health Aide (HHA)/PCA to join our team and provide exceptional care to our clients in the East Bronx. You'll work closely with our team to deliver high-quality care and support to patients in their homes, emphasizing patient-centered care and cultural sensitivity.
Responsibilities
- Provide personal care assistance, including bathing, dressing, and grooming, as needed
- Assist with daily living activities, such as feeding, toileting, and incontinence care
- Support mobility and ambulation, including transfers and walking
- Administer medication as ordered by a healthcare professional
- Assist with meal preparation, feeding, and hygiene
- Maintain accurate and detailed records of patient care
- Collaborate with healthcare professionals to develop and implement individualized care plans
- Provide emotional and psychological support to patients and families
Requirements
- Current certification as a Home Health Aide (HHA) or Personal Care Assistant (PCA)
- High school diploma or equivalent required
- Ability to work in a fast-paced environment with minimal supervision
- Strong communication and interpersonal skills
- Compassionate and empathetic attitude towards patients and families
