Alliance Care Rehabilitation & Nursing Center is seeking a dependable and compassionate Home Health Aide (HHA) Escort to support our residents during clinic and dialysis appointments. This role is ideal for HHAs who enjoy one-on-one resident interaction and want a consistent morning schedule while making a meaningful impact every day.

Responsibilities

• Safely escort residents to and from clinic and dialysis appointments
• Arrive one hour prior to scheduled clinic appointments
• Assist residents with preparation for appointments
• Ensure residents receive appropriate out-on-pass snacks
• Monitor residents closely to prevent falls or altercations and intervene as needed
• Maintain a full nursing uniform and wear ID badge at all times (on-site and off-site)
• Obtain clinic visit reports and ensure all required paperwork and supplies accompany residents
• Ensure residents’ DNR/DNI documentation and non-hospital DNR forms are present when required
• Advocate for residents during clinic visits and communicate concerns back to the facility
• Report all incidents or accidents immediately to the Clinical Coordinator or ADNS
• Represent Alliance Care Rehabilitation & Nursing Center professionally at all times
• Assist with additional clinic visits or unit duties as needed

Requirements

• Current New Jersey State Home Health Aide (HHA) certification required
• Ability to perform all duties of a Home Health Aide
• Strong communication skills (read, write, speak, and understand English)
• Knowledge of age-appropriate care and resident safety
• Reliable, compassionate, and professional demeanor

Pay

$16–$18 per hour
