Field-based, full-time home health aide position with Adaptive Home Health, a patient-centered, skilled home health provider that uses technology to reduce administrative burden and improve care access.
About the role
Adaptive Home Health is building a higher-acuity, technology-enabled home health model. Our tools—AI Scribe, prefilled assessments, photo-based medication extraction, and unified scheduling—cut documentation time so you can focus on patients. You’ll provide hands-on personal care, monitor conditions, and collaborate with nurses to deliver safe, dignified care in patients’ homes.
Responsibilities
- Provide personal care assistance: bathing, dressing, grooming, and toileting.
- Assist with safe transfers, ambulation, and mobility using proper body mechanics.
- Support patients with medication reminders as directed by the care plan.
- Monitor and report changes in patient condition, vital signs (when ordered), and daily living activities to the supervising nurse.
- Perform light housekeeping tasks related to patient safety and care (laundry, meal preparation, maintaining a clean environment).
- Document all care accurately and timely in Homecare Homebase (HCHB).
- Communicate clearly with patients, families, and the care team to ensure continuity of care.
- Follow infection control practices and maintain patient dignity and privacy.
- Assist with range-of-motion exercises and other restorative care activities as ordered.
- Coordinate with supervising RN/LVN for care plan updates and patient needs.
Requirements
- Current Texas HHA certification.
- 1+ years of experience providing personal care (home health, assisted living, nursing home, or hospital setting).
- CPR/BLS certification.
- Current Texas driver’s license and reliable transportation.
- Ability to lift, transfer, and provide hands-on care safely.
- Strong communication skills and professional demeanor.
- Comfortable working independently in patients’ homes.
- High school diploma or GED.
Nice-to-haves
- Previous home health experience.
- CNA or other clinical training/certification.
- Experience with medically complex or high-acuity patients.
- Bilingual (English/Spanish).

Pay
$17.00 – $18.00 per hour plus mileage reimbursement.
