About the Role
Acacia Care Rehab and Nursing Center is seeking a compassionate, dependable, and patient-focused Home Health Aide (HHA) to join our team on a part-time basis. In this role, you will accompany residents to medical appointments, dialysis treatments, and other off-site visits while ensuring their comfort, safety, and well-being throughout each outing.
Responsibilities
- Escort residents safely to and from medical appointments, dialysis treatments, clinics, and other off-site healthcare visits.
- Ensure resident safety during transportation and appointments by monitoring for fall risks and other safety concerns.
- Assist residents in preparing for appointments, including dressing and gathering personal belongings.
- Provide companionship, support, and snacks during extended outings or approved out-on-pass periods.
- Observe and promptly report any incidents, accidents, behavioral concerns, or changes in resident condition to the Clinical Coordinator or Assistant Director of Nursing (ADNS).
- Maintain professionalism while representing the facility during off-site appointments.
- Communicate effectively with residents, families, transportation providers, and healthcare professionals.
- Follow all facility policies, procedures, and infection prevention guidelines.
- Perform other duties as assigned to support resident care.
Requirements
- Current New Jersey Home Health Aide (HHA) Certification required.
- Previous experience working with geriatric or long-term care residents is preferred.
- Excellent communication and interpersonal skills.
- Compassionate, patient, and dependable with a strong commitment to resident-centered care.
- Ability to work independently while remaining a collaborative member of the healthcare team.
- Reliable attendance and flexibility to meet scheduling needs.
