Responsibilities
- Following individualized care plans and communicating any changes in condition
- Providing safety oversight
- Assisting clients with personal care (bathing, dressing, toileting, etc.)
- Providing support for safe mobility (experience with a Hoyer lift preferred)
- Providing companionship and emotional support
- Escorting clients to appointments, errands, or community activities
- Performing light housekeeping, laundry services, household organization
- Meal planning and preparation according to dietary needs
- Grocery shopping and errands
- Performing other duties as assigned within agency scope
Requirements
- HHA, PCA, or PCT certification
- CPR Certification preferred
- Follow a detailed care plan and complete daily reporting post shift
- Valid drivers license and clean driving record if transportation is provided as part of duties
- A valid driver's license is not a requirement to apply for the job or to be hired
- Must pass a criminal background check and motor vehicle driving record check
- Likely to have a legal authorization to work in the United States
- Must be 18 years of age or older
- Maintain client confidentiality and privacy
- Comply with CT elder abuse and neglect reporting requirements
- Have a compassionate and professional attitude with excellent communication skills
- Ability to pass a pre-employment drug screening
