About the role
The Dulla Care Home Healthcare HHS provides direct, comprehensive medical and non-medical care for patients and their families in an alternative setting. The role aims to support the reduction of hospital admissions, decrease ER utilization, improve patient success, and enhance the patient and provider experience.
Responsibilities
- Aids in the personal care of the client including oral hygiene, bathing, toileting, personal grooming, dressing, medication administration, range-of-motion exercises, transfers, turning, positioning, vital sign taking, ostomy care, weight measurement, intake and output monitoring, blood glucose testing, dressing changes, equipment cleaning, record keeping, laundry, bed making, room tidying, shopping, meal preparation, and assisting with feeding and menu planning.
- Communicates effectively with the patient, family, and care team to ensure patient needs are met and progress towards goals is achieved.
- Works collaboratively with a multidisciplinary team to develop and implement a patient’s plan of care.
- Maintains a clean and safe environment for the patient.
Requirements
- High School Diploma or GED
- State certification/licensure/training in Home Health, Choices Aide, STNA, or MA-C
- Minimum of 3 years acute care or home care aide experience
- BLS and ACLS certified
- Lift & carry equipment/supplies up to 50 pounds, push/pull objects up to 100 pounds, walk long distances, twist, bend, stoop/squat frequently, reach overhead/above shoulder level, below the waist, and in front of body, move within confined spaces, stand, sustain repetitive movement, maintain physical tolerance and balance in multiple positions, and drive a vehicle.
