Job Summary
POSITION IS OPEN UNTIL FILLED - FIRST REVIEW OF APPLICATIONS: JULY 13TH
About the role
Assists elderly clients or clients with disabilities by providing personal cares and assistance with minimal homemaking duties. May assist professional staff with community health outreach activities or events.
Responsibilities
- Personal care, light housekeeping and meal preparation.
- Assists with and/or helps to teach clients to perform the activities of daily living.
- Helps clients with prescribed exercises and treatments.
- Aids in medication management.
- Helps clients to plan, organize, and/or carry out household tasks.
- Promotes client's mental alertness through conversation and other assigned activities.
- Helps clients to maintain a safe living environment.
- Reports any changes in the client's mental or physical condition or family situation.
- Daily charting on client care, and weekly checking for client assignments.
- Fulfills Agency policies regarding infection control and data privacy.
- Fulfills the plan of care for each client as determined by nurse case manager.
- Assists professional staff with community health outreach activities or emergency preparedness activities.
Qualifications
- High school diploma.
- Certified Nursing Assistant/Home Health Aide or Licensed Practical Nurse. (CNA/HHA must be currently licensed on the Minnesota Department of Health Nursing Assistant Registry).
- Valid Minnesota driver's license.
- One to three years' work experience in a related field.
- Prior experience in Home Care or Long-Term Care Facility preferred.
- Bilingual (English/Spanish or English/Somali) preferred.
Supplemental Information
- HOURS: Variable. Typical work hours 20-30 hours per week. Monday - Friday from 8:00 a.m. - 3:30 pm
- Non-Exempt, General Unit Union – Grade 5
- MUST ATTACH CNA/ HHA CERTIFICATE TO ONLINE APPLICATION.
- Part-Time employees in a regular position may qualify for pro-rated benefits based on the number of hours worked.
