What You Will Do As An Admission Nurse
- Complete comprehensive Skilled Home Health admissions in the home setting
- Perform advanced clinical assessments across physical, psychosocial, and environmental domains
- Translate patient presentation into a clear, accurate initial plan of care
- Identify clinical risks, safety concerns, and gaps in care at the point of entry
- Ensure all admission documentation is complete, compliant, and clinically defensible
- Communicate findings and significant changes promptly to physicians and care teams (within 24 hours)
- Maintain accuracy and compliance aligned with internal standards and regulatory expectations
Requirements
- Associate or Bachelor’s Degree in Nursing preferred
- Valid driver’s license, insurance, and reliable transportation required
- Home Health experience
- Ability and willingness to travel up to 50% within the coverage area
- Able to sit, stand, bend, lift, and move intermittently; able to lift 50–100 lbs
