Home Care Speech-Language Pathologist (SLP)

About the role

We are seeking an experienced Home Care Speech Language Pathologist (SLP) to provide patient-centered care in a home health setting.

Responsibilities

  • Deliver direct treatment to patients with speech, language, swallow, or cognitive disorders.
  • Evaluate and develop individualized care plans for patients.
  • Document patient progress accurately and in a timely manner.
  • Communicate effectively with healthcare teams, attend conferences, and support education initiatives for professionals, patients, and the community.
  • Supervise and mentor peer and trainee performance.
  • Assist with the orientation of new staff.
  • Contribute to research activities.

Qualifications

  • Master’s degree in speech language pathology.
  • ASHA certification.
  • Current state licensure.
  • Strong knowledge of speech pathology principles.
  • Excellent communication and organizational skills.
  • CPR certification.
  • At least one year of relevant experience.

Requirements

  • Willingness to travel.
  • Minimum of twelve hours of continuing education annually.
  • Ability to work independently in various settings.
