About the Role
Our Home Care Physical Therapists (PT) use cutting-edge technology and clinical knowledge to provide exceptional one-to-one care to patients in their homes. Compassionate care is delivered through a multidisciplinary approach, collaborating with physicians, nursing team members, social workers, and home health aides. This role requires autonomy, high motivation, attention to detail, organization, and technological proficiency.
Territory coverage includes: Bedford Park, Bridgeview, Brookfield, Clarendon Hills, Countryside, Elmhurst, Hinsdale, Willowbrook, La Grange, Lyons, McCook, Oakbrook/Terrace, and Stickney.
Responsibilities
- Provide one-to-one care with patients in their homes
- Work in a patient-centered, supportive, and motivated environment
- Maintain flexibility in scheduling and patient needs
- Utilize advanced, easy-to-use technology for care delivery
Requirements
- Bachelor’s degree with progressive experience or Master’s degree in Physical Therapy
- Licensed to practice as a physical therapist in Illinois
- Current working knowledge of conditions of participation and home healthcare experience preferred
- Valid driver’s license and reliable transportation to and from work sites

Pay
Pay range: $38.81–$62.09 per hour
