Home Care Physical Therapist - $25k bonus

Key Responsibilities

• Carry out comprehensive assessments and develop individualized treatment plans.
• Deliver therapeutic interventions such as manual therapy, modalities, and exercise programs.
• Educate clients, families, and caregivers on therapy techniques and home programs.
• Supervise Physical Therapist Assistants (PTAs) and Home Health Aides (as permitted).
• Collaborate with interdisciplinary teams and participate in care conferences.
• Maintain accurate, timely documentation in compliance with regulations.

Qualifications

• Completed a degree from an APTA-accredited Physical Therapy program.
• Active state license to practice as a Physical Therapist.
• At least one year of experience or completion of a 6–8 week preceptorship.
• Experience in home health setting and knowledge of Medicare Conditions of Participation preferred.
