Shift: Per Diem; Days; 8a-4:30p
Responsibilities
- Performs evaluations and re-evaluations on patients to establish plan of care and goals.
- Performs patient treatment per Occupational Therapy procedures of modalities, therapeutic exercise, ADL training and other accepted methods.
- Communicates pertinent information, verbally or written, to physician, other staff members and patients/caregivers.
- Supervises and directs Home Health Aide’s care in relationship to the patient’s rehab program.
- Demonstrates active participation in home care department functions.
Qualifications
- EDUCATION, EXPERIENCE, TRAINING
- Bachelor’s degree or Master’s degree in Occupational Therapy.
- Current license as an Occupational Therapist.
- One (1) year recent clinical experience.
- Home Care experience preferred.
- Current BLS certification.
- Current driver’s license, car insurance, car registration, and reliable automobile.
