Job Summary
We are seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) for a home care position in Park City, UT. The ideal candidate will have at least one year of nursing experience, current unrestricted licenses in Utah, and certified CPR.
Responsibilities
- Medication set up and oversight
- Provide thoughtful care to clients based on personalized Plan of Care
- Provide in-home assessments
- Monitor and track changes in the clients' conditions and needs, and keep the Director of Nursing and any other appropriate personnel informed
- Collaborate with client's family on plan of care, health changes, and concerns
- Total personal cares
Requirements
- At least 1 year nursing experience
- Current unrestricted RN or LPN license in Utah
- Certified CPR Certification
- Negative TB skin test or chest x-ray
- Licensed driver with valid automobile insurance
- Ability to lift 75 pounds

Qualifications
The client is a 19-year-old male with a TBI who is wheelchair-bound, limited in speech, and hemiplegic. The ideal candidate should have experience in neurological or behavioral health.
Schedule
Available shifts include night shifts from 7pm to 7am, 2 or 3 nights per week in Park City.
