Home Care Nurse - RN - LPN

About the role

Client is a 19-year-old male with TBI. He is wheelchair bound, has limited speech, and is hemiplegic. Neurological and/or behavioral health experience is a must.

Available Shifts

• Park City location
• Night shifts: 7pm – 7am, 2 or 3 nights per week (Saturday, Sunday, Monday)
• Afternoon shifts: Monday – Friday, 3pm – 7pm

Responsibilities

• Medication set up and oversight
• Provide thoughtful care to clients based on personalized Plan of Care
• Provide in-home assessments
• Monitor and track changes in the client's conditions and needs, and keep the Director of Nursing and any other appropriate personnel informed
• Collaborate with client's family on plan of care, health changes, and concerns
• Total personal cares

Requirements

• Current unrestricted RN or LPN license in Utah
• Must have at least 1 year nursing experience with neurological and/or behavioral health
• Must have at least 1 year nursing experience in home
• Certified CPR Certification
• Negative TB skin test or chest x-ray
• Must be able to pass a drug test and a background check
• Licensed driver with valid automobile insurance
• Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull
• Must be able to lift 75 pounds
