Home Care Nurse (LPN/RN)

About the role

Provide one-on-one care for pediatric and young-adult clients in home, school, and summer settings, including Extended School Year (ESY) and school-based cases. Deliver high-tech ventilator and tracheostomy care while ensuring patient safety and comfort.

Responsibilities

• Complete paid training in-office, in-home, and online
• Utilize electronic charting systems
• Support clients with a variety of diagnoses and acuity levels
• Maintain compliance with healthcare standards and protocols

Requirements

• Valid U.S. nursing license
• Graduation from an accredited nursing program
• Experience with trach/vent care preferred
• Strong communication, critical-thinking, and adaptability skills
