About the role

Under the direct supervision of the Center Manager, the Senior Care Home Care Coordinator is responsible for the development and implementation of homecare services for program participants, including the coordination of all Durable Medical Equipment (DME) and community-based care.

Responsibilities

  • Assesses the home and personal care needs of the frail and elderly population and identifies and works with the Interdisciplinary Team (IDT) to develop specific plans of care.
  • Conducts initial and periodic assessments minimally every 6 months, completed prior to the scheduled team meeting.
  • Receives reports from home care staff and communicates participant changes to IDT members in a timely manner.
  • Schedules and coordinates home care staff—both contract and/or employed—for in-home care delivery and the implementation of all personal care services to ensure quality services are provided to meet participant needs.
  • Coordinates all DME provisions in the home setting; works with IDT on DME authorization requests.
  • Coordinates 24-hour care delivery.
  • Documents participant changes appropriately in medical records and communicates changes to IDT in a timely manner.
  • Supervises home care staff based in the community.
  • Manages human resources by determining qualifications, competence, and performance expectations of home care aides; implements and monitors staffing plans; provides orientation, in-service training, and continuing education of assigned staff; and applies policies/procedures pertaining to human resource management.
  • Stays up-to-date on home care standards and best practices.
  • Drives to community locations and residences to provide on-site training and competency checks of home care staff.
  • Provides effective leadership by coordinating and integrating services within the department, with other departments, and with the IDT’s primary functions.

Requirements

  • Five years’ experience in healthcare or related experience.
  • Valid driver's license.
  • BLS Certification required within 90 days of hire.
  • One year of experience working with a frail or elderly population. In absence of work experience, training may be provided upon hire.

Qualifications (Preferred)

  • BSN preferred.
  • Supervisory experience preferred.
  • Current valid RN license preferred.
