About the role

The Care Coordinator Supervisor represents the Agency as a frontline customer service liaison between patients/clients, caregivers, and the CSM. They work under the direction of the CSM and coordinate daily operations such as payroll and billing processes, customer support, personnel files, and medical records maintenance.

Responsibilities

  • Supports the needs of the patient/client by conferring with the CSM for direction and offering cases to qualified staff.
  • Maintains an active employee list of available caregivers to facilitate prompt assignment of staff to each case.
  • Ensures caregivers meet all Company policy requirements and are willing to work during compatible time frames.
  • Facilitates continuity of care by assigning the same caregiver to each patient/client whenever possible.
  • Schedules patient/client care according to the plan of care, ensuring only the amount, frequency, and duration of services requested.
  • Maintains and updates patient/client and caregiver lists weekly, documenting daily schedule changes.
  • Validates caregiver documentation against schedules and appropriately resolves any discrepancies.
  • Assists the staffing coordinator in meeting staffing needs, considering pay and bill rates and caregiver availability.
  • Contacts patients and families to confirm satisfaction after a new caregiver's first visit and may conduct customer satisfaction calls.
  • Hires caregivers by identifying recruitment needs, screening applicants, administering evaluations, and arranging interviews.
  • Manages medical supplies, including inventory, tracking, distribution, auditing usage, billing, and physician orders.
  • Maintains and audits client records and clinical files in SmartCare, and communicates identified discrepancies or problems.
  • Supports the Client Services Manager in marketing the business through market research, reviewing successful campaigns, and participating in team efforts.
  • Answers the Agency's phone lines, provides clerical support to the Staffing Coordinator, and handles incoming and outgoing mail.
  • Participates in payroll processes, prepares and validates bills for all payers, and supports scheduling patient visits.
  • Maintains personnel files, audits file accuracy, and reports incomplete, outdated, or inaccurate information to the Client Services Manager.
  • Maintains a safe environment for all clients/patients, physicians, and employees, and provides excellent customer service.
  • Supports and participates in performance improvement processes and assumes responsibility for all mandatory in-services, certifications, or licenses.

Qualifications

  • Bachelor's degree in healthcare, social work, or a related field.
  • Minimum 3 years of experience in home health care, patient care, or related field.
  • Knowledge of agency policies and procedures.
  • Excellent communication and interpersonal skills.
  • Ability to manage multiple tasks and prioritize responsibilities.
  • Proficiency in Microsoft Office Suite.

$50,000 USD per year.
