Job Summary

As a Home Care Associate, you will play a vital role in supporting our caregivers, patients, and families by providing exceptional customer service, administrative assistance, and ensuring the smooth operation of our daily activities.

Responsibilities

• Provide exceptional customer service to patients, families, and caregivers over the phone and in-person
• Aid with scheduling, coordinating, and processing of care assignments and patient data
• Perform administrative tasks, including data entry, filing, and document management
• Collaborate with caregivers, nurses, and other healthcare professionals to ensure seamless care delivery
• Participate in team meetings, training sessions, and quality improvement initiatives
• Maintain accurate and confidential records, adhering to HIPAA guidelines

Requirements

• Strong communication and interpersonal skills
