St. Peter’s Health Partners is seeking a full-time Nurse Practitioner to join our Home Based Primary Care Team, delivering high-quality primary care directly to patients in the Eddy Senior Living Communities throughout the Capital Region. Since 2013, our specialized team has served vulnerable individuals who are unable—either temporarily or long-term—to access care in traditional practice settings. This model provides exceptional care while achieving high patient and caregiver satisfaction, improving health outcomes, reducing avoidable ER visits and hospitalizations, and delaying or preventing nursing home placement. About the Role
This is a full-time Home Based Primary Care Nurse Practitioner role providing primary care services directly to senior living patients in the Eddy Senior Living Communities. The position features a home-based schedule with start/end from home, no weekends or holidays, and minimal on-call duties. Patient panels are significantly smaller than a typical primary care practice (150–185 patients), with approximately five scheduled visits per day, allowing time for travel and longer visits due to the complex needs of the patient population.

Requirements
- Board Certified Adult Gerontology or Family Nurse Practitioner certification (Internal Medicine also accepted)
- New York State license and DEA certification
- At least 2–3 years of Nurse Practitioner experience working with an elderly population (required)
- Experience working as a Nurse Practitioner in a primary care setting (preferred)
Pay
Compensation range: $124,800–$139,360, plus on-call pay and mileage reimbursement. The posted salary reflects the starting range of total compensation and does not include productivity bonuses, incentives related to quality and performance, extra shift incentives, or other forms of cash compensation as applicable to the position.
