HIV and Prevention Coordinator

About the Role

The HIV and Prevention Care Coordinator plays a critical role in supporting patients living with HIV or at risk of HIV acquisition. Working closely with clinical care teams, you will assist patients in obtaining timely, essential, and appropriate HIV and preventative medical and psychosocial services to optimize health outcomes and prevent HIV acquisition. Responsibilities include linkage to care, engagement interventions, navigation support, adherence counseling, follow-up outreach (in person, by phone, or electronically), and liaising with clinical service providers to address specific health concerns. Additional duties involve assisting with the development and implementation of HIV care and preventative protocols, reviewing patient charts, and tracking patient data via electronic health records and other systems.

Department

Ward 86, the HIV/AIDS Clinic at Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG), has been a leader and innovator in HIV care since the beginning of the epidemic. As the largest clinic providing primary medical care for people living with HIV in the San Francisco Health Network (SFHN), Ward 86 also offers preventive services, including pre-exposure prophylaxis (PrEP), to patients at risk for HIV infection. The clinic serves approximately 2,500 patients living with HIV, with over 24,000 clinical encounters annually. Services are delivered through a comprehensive, multidisciplinary approach, addressing primary medical and HIV care, urgent care, mental health evaluation and treatment, substance use assessment and referrals, HIV prevention, specialty care (including on-site Pulmonary, Dermatology, and Cardiology clinics), and psychosocial interventions.

Responsibilities

• Assist patients with linkage to HIV and preventative care services.
• Provide engagement interventions for patients at risk of falling out of care.
• Navigate patients through the healthcare system and address barriers to care.
• Conduct adherence counseling to support treatment compliance.
• Perform follow-up outreach in person, by phone, or via electronic means.
• Liaise with clinical service providers to address specific health concerns.
• Assist in the development, modification, and implementation of HIV care and preventative protocols.
• Review patient charts and enter/track patient data in electronic health records and other data systems.

Requirements

• Bachelor’s degree in a related area and three or more years of relevant experience, or equivalent experience/training.
• Strong knowledge of Patient Rights & Responsibilities, Joint Commission standards, and Centers for Medicare/Medicaid regulations.
• Familiarity with medical terminology.
• Proficiency in data collection, compilation, and analytical techniques.
• Ability to assess patient grievances and quickly locate appropriate resources for assistance.
• In-depth knowledge of organizational processes to resolve issues effectively.
• Excellent customer service skills, with experience working in high-volume, high-expectation environments.
• Ability to support patients with diverse clinical and non-clinical needs.
• Experience working in complex, multi-divisional environments, with skills to establish priorities and achieve goals.
• Proficiency with project management tools and Windows-based software (Microsoft Word, Excel, Outlook).
• Knowledge of computer systems and software used in the functional area.
• Experience working with ethnically, culturally, and sexually diverse populations.
• Bilingual in English/Spanish or English/Cantonese (speaking, reading, and writing).
• Ability to travel and work among various clinics within San Francisco.
• Strong organizational skills and attention to detail.
• Excellent oral and written communication skills.

Preferred Qualifications

• Experience working with people living with chronic illness, substance users, or persons with mental health diagnoses.
• Knowledge of HIV/STD treatment and prevention, with a focus on harm reduction.
• Work experience in a healthcare organization.
• Ability to quickly learn Electronic Medical Records (EMR), disease registry, and other information systems.
• California HIV Test Counseling Certification.
