Duties/Responsibilities

• Obtain vital signs, record them, and notify the office of deviations according to standard practice.
• Provide personal hygiene services such as bathing, grooming, dressing, hair care, shaving, deodorant application, skin care, nail care, and oral hygiene.
• Change bed linens and make the bed.
• Eat and maintain adequate nutrition and fluid intake when appropriate.
• Ambulate and transfer patients as necessary, including assisting with toileting, bedpan use, and transfers.
• Perform incidental household services such as light cleaning and laundry.
• Remind patients about medication schedules and assist with taking medications as directed.
• Understand and apply basic infection control principles and report any concerns to the supervisor.
• Follow the Plan of Care as instructed by the Agency's health care professional and document appropriately.
• Monitor and report any changes in the patient's condition or family situation.
• Contact the office promptly if unable to visit a patient as scheduled and provide a full report.
• Communicate effectively with patients, families, and Agency personnel.
• Attend required in-services and meetings.
• Participate in Performance Improvement activities as indicated.
